 bearing analysis: recurring BPFO on motor drive end even if new motor is isntalled

Good day to all! we have recurring problem in our major fan motor, even new motor was installed i can still detect raised noise floor and BPFO. I attached the bearing of the pulled out old motor, Based on the bearing pattern, what is the usual generic cause? I  am aware that there is some degree of bearing outer ring movement and is easier to address. My concern is the cloud white on outer race what are the common cause.

Here are a couple of resources, in my opinion, very loose mechanical fits causing raceway to deform under heavy load. Lubrication contamination from the loose fits and possibly how the bearings are greased. Potential electrical discharge can causing frosting of ball path on outer or inner race.
